sp_who will give you loginname & hostname. isn't this what you want ?

| Sorry. the same i wanted.I have one more query when i see the SQL built in reports like "Schema Changes History" Its Only Showing login name as "SA"Can I able to see HostName/Machine Name from where the Objects have been modified. |
