 rebuild AND reorganize index?

I'm having a little trouble understand which of these two is appropriate for an application. The application in question has a lot of reads and writes. Data is constantly inserted and updated.

Currently we have been executing a nightly maintenance plan to rebuild the indexes, then reorganize them. Are both steps necessary for most efficient query results?

NO. Reorganizing only organizes the leaf level of indexes while rebuilding does everything including updating stats. You have to see fragmentation level and do reorganize or rebuild index.
