I have a legacy SQL Server 6.5 database that I need to get the data out of. I am going to be importing the data into MySQL. How can I export csv or insert statements along with the create table DDL?

SQL Server 6.5's Enterprise Manager has a "Generate SQL script" wizard that will allow you to generate the schema script. You can then use bcp.exe to export the data to csv files.
