I have the following backup and delete job:

What I want to do is have only one backup file per database. I've created a cleanup job but currently I have 4 backup files per database so I assume I made something wrong. What should I do?

Are they about the same size? What are the file extensions? Are the timestamps over 4 days or 2 days?

The task you included does not cleanup the backup files - it cleans up the history tables in MSDB. You need to add the maintenance cleanup task and configure that to cleanup the files.
