help very urgent .......very very urgent.

i have data below:

table_name col_count date_col

t1 20 10/11/13
t2 21 10/11/13
t1 25 10/12/13
t2 27 10/12/13

I need to display like:

table_name 10/11/13 10/12/13
t1 20 25
t2 21 27

please help it's urgent.
