My personal favorites are the optical illusion costumes.

One year I stole an idea from the Benny Hill show where I made a fake old-lady torso and fake legs that looked like mine. I dressed like an old lady from the waste down so that the overall impression was that an old lady was carrying me around on piggy-back. Of couse it was my legs that were actually doing the running and jumping around. My friends said that it worked great and I got many laughs although to my "adult-oriented" friends it looked kind of perverted.

What I usually end up doing is something much quicker. I use some shaping puddy and make up to create a nasty open head or neck wound. My mother's birthday is on halloween and one year I did a particularly nice head wound. And sticking out of the wound was a tiny human arm that had a sign clutched in it's fist that said "Happy Birthday, Mom!" That got a lot of attention when we were out to dinner.

There is a photo of Graz dressed as a nun at work from a few years back

He hasn't made a habit of it though.

"optical illusion costumes"

Yeah, I did that once. Fancy dress party skiing. I had a Gorilla carry "me" in a cage.

Wifey was not impressed with the three suitcases my costume and cage took, which meant we had no baggage allowance for anything else!

People thought I was mad, so I rate it as a success!
