I have SQL Server 2005 Version 9.00.1399.06, also known as RTM. It is just installed out of the box, what patches and fixes should be applied to make sure SQL Server is at optimal performance and security?

Install latest service pack. To this date I think it is service pack 3.

Does SP3 include all updates from SP1 and SP2? In other words, does one need to install SP1, SP2 and SP3 or just SP3 will do?

Yes SP3 is cumulative. There is already a cumulative update for it though, so be sure you also install CU1 so that you are completely up to date.
