 Which folder do you put your mdf/ldf files?

If you don't specify anything, the default folder for the data files go into c:\Program Files\..\..\mssql\data\. Which may not be the best place for data files.

Which/what folder structure do you put your data files under?

Generally, just:

[drive letter of choice]:\mssql\data
[drive letter of choice]:\mssql\index
[drive letter of choice]:\mssql\logs

I avoid chosing the drive which has windows installed.

And for this setup, it's ideal that they are separate PHYSICAL drives, not simply partitions of one physical. It's all about performance!

We use 4 mount points, so although it looks like they are on the same disks, they aren't. Here's our structure:

F:F:\BackupF:\DataF:\Log
